What is Eye Surgery?

Also known as ocular surgery, eye surgery is conducted by an ophthalmologist. The necessity for eye surgery is determined by various conditions. Whether it's corrective procedures or orbital implants, these treatments offer successful solutions to a range of eye and vision problems.

Cataract Surgery

Cataracts typically impact both eyes and are more prevalent in adults aged 60 and above. The cataract surgery procedure is highly successful and safe. It is recommended for individuals experiencing vision loss that significantly affects their daily life. However, those with other concurrent eye diseases may not be suitable candidates for the surgery. The procedure lasts approximately thirty minutes and is conducted under minimal sedation. During surgery, an artificial lens is implanted to replace the original, restoring vision.

The 3 approaches of the cataract surgery are :

Extracapsular Cataract Surgery : The surgical procedure is carried out when the lens is too opaque for Phacoemulsification. This surgery involves suturing, and the recovery period is relatively longer.

Intracapsular Cataract Surgery : The surgical procedure entails creating substantial incisions and completely removing the surrounding capsule and the entire lens. This method is rarely employed.

Phacoemulsification : The surgical procedure is carried out under local anesthesia. In the most common type of cataract surgery, ultrasonic vibrations are utilized to dissolve the cloudy lens by inserting a probe.

Corrective Surgery

This is the most common and popular eye surgery. PRK (Photorefractive keratectomy) and LASIK (laser-assisted in Situ Keratomileusis) are two common methods of corrective eye surgery. Laser eye surgery can correct hyperopia, astigmatism, and myopia, eliminating the need for corrective lenses.

  • PRK is recommended for individuals with thin corneas, as it can be challenging to create a flap in such cases.
  • LASIK is the most commonly performed procedure, utilizing precision laser to remove corneal tissue. The laser reshapes the cornea, adjusting its focus level.

Orbital Surgery

The surgery is intended for conditions such as enucleation, evisceration, and anophthalmia. Orbital surgery is recommended for individuals with large eye tumors, congenital defects resulting in absent eyes, and eyes that cannot be protected due to trauma. Two types of orbital implants are silicone and bioceramic orbital implants, along with the use of orbital tissue expanders. These implants serve as surgical approaches for the procedure, playing a role in preserving the natural structure of the eye and providing support for the positioning of an artificial eye.

Glaucoma Surgery

Glaucoma can result in vision loss by increasing intraocular pressure, and surgery is often conducted to reduce this pressure. The procedure can be performed multiple times with low risk. Types of glaucoma surgery include :

  • Conventional Surgery : The surgery involves creating a new drainage canal to facilitate the drainage of intraocular fluid out of the eye.

  • Laser Surgery : The surgery involves creating a small opening in the eye's drainage system using a laser. This aids in improving the drainage of fluid from the eye.

Lip Repair Surgery

Ptosis surgery can be performed for either functional or cosmetic reasons. Lid repair surgery is used to address the condition of droopy eyelids (ptosis). Various surgeries are employed for ptosis, including Fasanella-Servat, Frontalis suspension (for severe congenital cases), Muller’s muscle-conjunctival resection (for mild to moderate ptosis), and Blepharoplasty (for ptosis due to aging).

Presbyopia Surgery

Presbyopia typically sets in between the ages of 40-50, causing an inability of the eye to focus at various distances. This condition becomes noticeable when individuals find print becoming blurred. The stiffening of the eye's lens is a common cause of Presbyopia. It is also associated with the atrophy of muscles controlling the lens or changes in the lens itself. Correcting Presbyopia can involve creating 'monovision' during LASIK. Conductive keratoplasty is another method for correcting this common vision condition.

Squint Eye

Squint, also known as strabismus, occurs when both eyes are not aligned in the same direction. It can manifest as either convergent squint (turning in) or divergent squint (turning out). Vertical squint involves one eye being positioned either lower or higher than the other. Squint can be either continuous or occasional. Premature babies, particularly those who have been ill at a very early age, may develop a squint, especially if there is damage to the brain. Reasons for the development of these squints include :

  • Poor vision can lead to the brain's inability to coordinate and use both eyes together, resulting in the development of squint.
  • Poor vision in one eye, especially in childhood, may necessitate the use of glasses. The specific condition could be myopia (short-sightedness), astigmatism (abnormal shape of the front of the eye), or hypermetropia (long-sightedness).
  • A scar on the front or the presence of a cataract can contribute to poor vision in the eye. Additionally, it is rare, but there are cases where one eye may not have developed properly due to issues in the optic nerve or retina.
  • Squint can rarely develop in short-sighted individuals but is more common in those who are long-sighted. Children with long-sightedness may develop a convergent squint, as they exert more effort to focus.
  • The abnormal development of muscles or nerves responsible for controlling eye movement could be another contributing factor.

Refractive Eye Surgery

Refractive errors such as hyperopia (farsightedness), presbyopia, myopia (nearsightedness), and astigmatism can be addressed through refractive surgery. This surgical procedure enhances and rectifies vision by eliminating these refractive errors. The surgery aims to enhance the eye's focus either by creating a clear circular dome in front of the eye or by reshaping the cornea.

Eye Cancer

Eye cancer refers to the formation of tumors in various parts of the eye. This condition occurs when cells in and around the eye undergo uncontrolled growth and changes, resulting in the formation of a mass or tumor. These tumors can be either cancerous or non-cancerous. Intraocular malignancy specifically refers to cancer developing within the eyeball.

Intraocular Cancer Types

Uveal metastases are recognized as the most prevalent form of intraocular cancer in adults, originating in other parts of the body and spreading to the uvea. The predominant primary intraocular cancer observed in adults is melanoma, resulting from the uncontrolled growth of melanocytes, the pigmented cells within the eye. Intraocular melanoma is also referred to as uveal melanoma.

The other types of cancers are :

  • Retinoblastoma
  • Conjunctival Melanoma
  • Lacrimal Gland Tumor
  • Intraocular Lymphoma
  • Eyelid Carcinoma
  • Hemangioma
