What is Tympanoplasty?

Tympanoplasty, a surgical intervention, aims to reconstruct a perforated eardrum (tympanic membrane) or the small bones in the middle ear. The issue with the tympanic membrane may arise from a serious infection or any trauma affecting the eardrum.

The tympanic membrane comprises three layers, with both inner and outer layers featuring epithelial cells. Perforations can occur due to defects in the middle ear, involving elastic collagen fibers. Smaller perforations typically heal easily, but larger defects may impede the healing process. Causes of perforated eardrums include traumatic events such as ear slaps, explosions, or foreign objects in the ear. Tympanoplasty surgery aims to repair the perforated eardrum and restore the middle ear bones, including the stapes, incus, and malleus.

Tympanoplasty Procedure

The surgical process can be conducted through the transcanal approach (ear canal), commencing with an incision either through the ear or at the back of the ear. To reconstruct the tympanic membrane, a graft is harvested, commonly from the tragus or temporalis fascia. The surgery is performed under local anesthesia and typically lasts 2-3 hours, with the duration influenced by the chosen approach. The procedure is carried out on an inpatient basis, boasting a success rate as high as 80-90%.

Tympanoplasty Myringoplasty

The human ear consists of three parts. The initial segment is the outer ear, responsible for collecting sound. The middle ear connects to the outer ear on either side of the head. Deeper inside, there is also an inner ear on each side. Positioned between the outer and middle ear, the eardrum spans the deepest part of the ear canal. When children have a hole in the eardrum, it leads to perforation. This condition may result from :

  • Using cotton swabs to clean the ears can cause trauma to the eardrum.
  • Frequent ear infections
  • Early insertion of ear tubes to facilitate drainage of fluid accumulating behind the eardrum.
  • Germs can enter through the hole, potentially leading to ear infections.

The presence of a hole significantly impacts hearing by disrupting the normal vibration of the eardrum. Sealing the hole can enhance hearing and prevent ear infections. The degree of hearing improvement is contingent upon the size of the hole.

Ear Lobes

Earlobes primarily consist of soft skin and a small amount of fatty tissue. Tears in earlobe tissues commonly result from trauma or excessive weight. To conceal the unappealing appearance of a torn earlobe, clip-on earrings can be used. Earlobe repair surgery may be necessary for various reasons, including :

  • Congenital ear lobe defect
  • Accident
  • Wearing heavy earrings can cause the hole in the earlobe to split or enlarge
  • Pulling earrings in a harsh way ( entanglement of earring and earring snatching attempt)
  • Stretched earlobe either by wearing heavy earring or by any other reason
  • Piercing of the ear where in a number of cases come with damaged keloid and earlobes
  • Some diseases
  • Sports mishap like wrestling

Ear Lobe Surgery

The surgery can be performed in a single session or on a stage-by-stage basis, depending largely on the extent of damage to the earlobe.

  • To address elongated earlobes resulting from wearing heavy earrings, stretched earlobe surgery is undertaken for repair.
  • Reconstructive earlobe surgery is carried out to address a damaged earlobe, correct defects present since birth, and reshape the earlobe.
  • Torn earlobe surgery is conducted to mend a tear in the earlobe.
  • Split earlobe surgery is undertaken to repair a split in the earlobe.

Octoplasty Surgery (Ear Pinning Surgery)

The surgical procedure aims to rectify disfiguring injuries or deformities affecting the external ear. Commonly referred to as ear pinning or ear repairing, this surgery is more frequently performed in children than in adults. It can provide a more aesthetically pleasing shape to the ears. Otoplasty, a cosmetic surgery, is utilized to reduce the size of overly large ears and reposition them closer to the head. Additionally, this surgery can address other issues such as :

  • The surgery is intended for individuals who have either lost their ears due to injury or were born without them.
  • Stretched or large earlobes
  • Lop Ear occurs when the tip of the ear folds down forward.
  • Cupped Ear (very small ear)
  • Shell Ear develops when the natural creases, folds, and the curvature in the outer rim are absent.
  • Lobes with wrinkles and large creases
